Note:
After all samples have been added, the internal header structure must be updated
by calling bcf_hdr_sync(). This is normally done automatically by the first bcf_hdr_write()
or bcf_write() call. Otherwise, the caller must force the update by calling bcf_hdr_sync()
explicitly.
